S3 DTP & Meningitis Vaccines Consent Form

Dear Parent/Carer

Your son/daughter will receive their DTP & Meningitis immunisation consent form on Tuesday 20th February during tutor class time.

If you wish your son/daughter to receive this immunisation, please complete the consent form and return to tutor teacher before Friday 8th March 2024.

Pupils who do not have a completed consent form will not be permitted to receive their immunisation.
