S1 - HPV Immunisation, Consent Form
Dear Parent/Carer
Your son/daughter will receive their HPV immunisation consent form, tomorrow, during tutor class time.
If you wish your son/daughter to receive this immunisation, please complete the consent form and return to tutor teacher before Wednesday 17th January 2024.
Pupils who do not have a completed consent form will not be permitted to receive their immunisation.
