Flu Immunisation Consent Form
Dear Parent/Carer
Your son daughter will be issued with their Flu Immunisation consent form, over the next couple of days, during tutor class time.
If you consent to your child having this immunisation, please sign the consent form and return it asap, to your child’s tutor teacher – but before Thursday 7th September 2023.
If your child did not receive an immunisation form, by the end of school time on Thursday, and you wish them to have this, please contact the following team:
